Application of pixel laser (2940 erbium laser) combined with FGF-7 spray in the treatment of facial rejuvenation

Considering that the previous treatment methods are not ideal for solving the complex problem of facial skin aging, and there are many side effects, our department adopts the method of 2940 erbium laser combined with FGF-7 spray to treat and observe 68 outpatients. The curative effect is accurate, the operation is simple, the patient compliance is good, the satisfaction rate is high, and there are few side effects, which is worth popularizing.

  1. Clinical data

General data: there were 68 patients with various skin problems caused by facial aging, including 6 males and 62 females. The age ranged from 33 to 47 years, with an average age of 37 years, including 3 cases of type II skin and 15 cases of type III skin. All patients completed 5 treatments.

Instruments and medicaments: Alam laser Ltd Ceasarea Israel, 2940nm erbium laser treatment hand tools and 570nm photon treatment hand tools are used. 9mm pixel laser (2940 erbium laser) hand tool × 9mm size optical head, i.e. 9mm for one treatment spot × 9mm, long pulse width and 800 ~ 1100mj/cm2 energy density.

  1. Method

Preoperative preparation: routine signature and photography before treatment. The same camera, the same position, the same light source, positive position and oblique position (45 degree angle) are used for photography. After cleansing with facial cleanser, enter the laser room for treatment.

Treatment method: Pixel laser (2940 erbium laser) was used for treatment. The next treatment was performed after an interval of 3 weeks. The total course of treatment was 5 times. The main treatment sites were eyebrow center, forehead, temporal part, cheeks and nose. Before treatment, lidocaine gel was applied to the operation area, and the surface was covered with film for half an hour. After anesthesia, chlorhexidine was used for local disinfection. The appropriate energy density was selected according to the location. 800mJ was selected for the crow’s feet around the eyes, 1000 ~ 1100mj for the nose and forehead, and 900 ~ 1100mj for the cheek. Each spot is repeatedly excited for 4-6 times, and the spots do not overlap.

Postoperative care: after the treatment, the face is sprayed with refrigerated FGF-7 spray and dried naturally. The patient does not use any other drugs or facial mask at the back of home. He uses the spray twice a day for a total of 7 days. If the burning sensation is obvious on the day of treatment, he can spray it at any time. Put the spray in a 4 ℃ refrigerator for cold storage. Repeat this step after each treatment. Do not touch the face with water 48 hours after treatment. After 48 hours, clean the face normally, apply skin care products, and apply sunscreen and sunshade during the whole course of treatment. If a small amount of exudation occurs locally, it is generally not subject to special treatment until it naturally scabs and falls off.

Evaluation of curative effect and side effects: the three doctors compared the positive and lateral photos of the operation area before the first treatment and after the fifth treatment, and evaluated them from the four aspects of color spots, pores, skin color and wrinkles. They were divided into five levels: level 0: no improvement, level 1: improvement of 1% ~ 24%, level 2: improvement of 25% ~ 49%, level 3: improvement of 50% ~ 74%, and level 4: improvement of 75% ~ 100%. The treatment was effective if the improvement reached level 3 or above. The statistical effective rate. At the same time, after each treatment, ask whether there are side effects after the last treatment, such as blisters, exudation, pigmentation, severe pain, allergy, etc., and record them on the medical record. If there are, give corresponding symptomatic treatment, and record the incidence of side effects.

All patients had self-assessment before treatment, after 5 times of treatment and after 6 months of follow-up. According to self satisfaction, they were divided into 5 levels: a level was dissatisfied; Level B means some satisfaction; Level C is satisfactory; Level D means very satisfied; Level E is very satisfactory. The total satisfaction rate was calculated by taking grade C and above as treatment satisfaction.

  1. Results

All patients completed 5 treatments. After evaluation by doctors, 49 patients reached grade 3 or above, with a total effective rate of 72.1%. It is considered that this treatment can significantly improve the skin aging of patients, such as coarse pores, color spots, small wrinkles, abnormal pigments, etc., and the satisfaction rate is high. Before treatment, all patients were dissatisfied with their facial aging. After treatment, the satisfaction rate was 95.6%. After 6-12 months of follow-up (telephone and outpatient follow-up), the satisfaction rate was still 92.6%. Patients generally believe that the curative effect is stable, the skin texture is significantly improved, the skin color is even and shiny, the recurrence of color spots and wrinkles is not obvious, and the recovery is fast after treatment. After spraying spray, the comfort is significantly increased. No serious side effects such as blisters, large-area pigmentation and allergy occurred.

  1. Typical cases

The patient is a 38 year old female with type III skin, mixed skin, obvious oil discharge from T-shaped area, large pores in the nose and the cheeks near the nose, more small wrinkles near the outer canthus, pigmentation after acne on the cheeks, and dark yellow skin.

  1. Discussion

Skin aging mainly includes natural aging and photoaging. Natural skin aging caused by aging is called endogenous skin aging. Skin aging caused by smoking, sunlight, air pollution, exposure to harmful chemicals and other external environmental factors can be classified as exogenous aging. Among them, excessive ultraviolet radiation is the main factor causing exogenous skin aging, which is called skin photoaging. Facial skin aging has multiple manifestations, including abnormal skin pigments (pigmentation, solar freckles, age spots, etc.), vascular damage (telangiectasia or disappearance), changes in the structure of epidermis and dermis (wrinkles, decreased skin elasticity and texture, coarse pores, changes in collagen fibers and elastic fibers).

Histologically, skin aging is mainly manifested in the structural changes of epidermis and dermis. The epidermal layer showed that the renewal of epidermis was slowed down, the barrier function was weakened, the activity of keratinocytes was decreased, and the repair ability of epidermis was weakened after injury.

The aging of dermis shows that the thickness of dermis becomes thinner, the synthesis of collagen and elastin decreases, and the decomposition increases. The ratio of type Ⅰ and Ⅲ collagen is gradually inverted, and the collagen fibers are arranged in disorder. Clinically, the main manifestations are wrinkles, large pores and sagging skin. Lipofuscin increased in the dermis, showing a variety of pigmentation and age spots.
